Mail the original cash register receipt(s), all cardboard pieces from five blister cards including the bottom cardboard piece with the UPC code, with plastic windows removed, for each vehicle you are requesting (limit of 10 vehicles per household), and the COMPLETED ORDER FORM (fill out the form and print it), along with $5.00 US shipping and processing for each vehicle you order (check or money order only, made payable to MATTEL) to:
